Transition checklist

  • Write the home program in the family's words, not clinical language.
  • Name the three tasks that carry the most function right now.
  • Set the next reassessment date and the exact tasks you will film.
  • Decide which orthotic or taping strategy continues and for how long.
  • Hand the caregiver one thing to do daily — not five.
  • Book the next check-in before the last session ends.
